Question: What is this horror movie?

I saw it a long time ago, it was about a man who worked at a mental hospital. There was a patient (I believe her name was Agnes or something like that), and she killed some of the staff at the mental hospital and escaped and started to haunt the man and his family. Does anyone know this movie? It may be a little bit older, but not too old. 10 points for best answer! Thanks.
